  • The organization that commissioned the artwork.
  • Artwork owner(s).
  • Select from options that best describe your work.
  • List any additional artwork types that best describe your artwork.
  • Detail the materials used in this artwork.
  • Select a placement type from the dropdown list to describe the site of the artwork.
  • List the complete street address of the artwork. Include latitude and longitude if possible.
